What are the application procedures for National Treasury check reissuance in the case of a lost check?

When the payee or holder loses a National Treasury check, the procedures to apply for the reissuance of the National Treasury check are as follows:
1.    If the National Treasury check is not more than one year past the issue date:
(1) The payee or holder, in accordance with the regulations, shall carry out the procedures for loss reporting and stop-payment with the National Treasury agent bank. 
(2) After the NTA has received a copy of the Notification of Loss Report and Stop-Payment of a Negotiable Instrument enclosed by the National Treasury agent bank, and has investigated and ascertained that the check has not yet been redeemed, the payee or holder may complete an Application for Reissuance of National Treasury Check and attach the identification documents of the applicant to the NTA.
2.    If the National Treasury check is more than one year past the issue date and the check has not been paid to the National Treasury:
The payee or holder shall apply to the NTA for loss reporting and stop-payment, and complete an Application for Reissuance of National Treasury Check and attach the identification documents of the applicant for its reissuance. After having duly completed the procedures for loss reporting and stop-payment, the NTA will thereupon, in accordance with the regulations, reissue the National Treasury check. 
3.    If the original payee or holder loses a National Treasury check and has already applied to a court for public summons procedure, once 30 days have elapsed from the date of the court's judgment of abridgement of rights declaring the lost check void, the payee or the holder shall complete an Application for Reissuance of National Treasury Check and attach the court judgment to apply to the NTA for reissuance.
