The procedures to apply for the replacement of the National Treasury check are as follows:
1. If the payee or holder completes an application for the replacement of a Treasury check with the original check and a copy of the applicant's identity document due to the circumstances that the handwriting on the face of the check is blurred, or stained, or the check is more than 1 year past the issue date and has not yet been paid to the National Treasury, if the NTA has verified that the application is in order, it shall issue a replacement National Treasury check with the original payee as the payee. When the payee or holder is a government agency or government-owned enterprise that applies for the replacement, it shall include a note on the Application for Replacement of National Treasury Check stating “If there is any dispute, this agency (enterprise) is responsible for handling it” and shall affix the seals of the agency. If necessary, a certification document may be provided by its superior competent authority.
2. If the original payee is deceased or factually cannot be found, the holder may apply for a replacement of the National Treasury check with the holder as the payee, who shall not only complete an application for the replacement of a Treasury check with the original check and a copy of the applicant's identity document, but also attach the certification documents issued by the agency that originally prepared the payment voucher.
